- Pitting Edema
1+/+1 – 2mm depth
2+/+2 – 4mm depth
3+/+3 – 6mm depth
4+/+4 – 8mm depth
- Tonsil Grading
1+ visible
2+ halfway between the tonsilar pillar and the uvula
3+ tonsils are touching the uvula
4+ tonsils are touching each other
- Pulse
0 absent, not palpable
1+ pulse diminished, barely palpable
2+ easily palpable, normal pulse
3+ full pulse, increased
4+ strong, bounding pulse, can’t be obliterated 
- Muscle strength
0 no contractility
1 slight contractility, no movement
2 full ROM, gravity eliminated
3 full ROM, with gravity
4 full ROM, against gravity some resistance
5 full ROM, against gravity full resistance
- Glasgow Coma Scale

2+ Normal with visible muscle twitch and movement of the arm and leg
3+ brisker than normal, may not indicate disease
4+ hyperactive very brisk; often associated with spinal cord injuries
